Do you have a favorite teacher? News 12 New Jersey along with Teachers' Insurance Plan of NJ wants to honor your favorite teachers during Teacher Appreciation Week!
Teachers' Insurance Plan of NJ will award $500 gift cards to five lucky teachers, chosen at random.
The submission period to nominate your favorite teacher is March 29 through 1 p.m. on April 23.
All you need to do is fill out the information below and upload a photo of your favorite teacher.
